The Professional Partner Program at Region 6 Behavioral Health has an in-home case management program that works with families who have children with unmet emotional/behavioral needs. The Professional Partner will assist children/adolescents and their families to achieve goals of stability, improved functioning and community integration. The Professional Partner must be willing to work a flexible schedule which includes some evenings and be responsive to crisis situations. A Bachelor's degree in Social Work or another related field is required plus two years paid full-time experience in providing direct service to SED children. Provides coordination services to assigned youth and their families utilizing the Wraparound, Family-Centered Processes; identifies and evaluates behavioral, medical and pharmacological interventions in order to assess appropriateness for service recommendations. Participate in a collaborative effort with existing systems such as school, juvenile justice, behavioral health, mental health, Child Protective Services, Health and Human Services, probation, etc. Bilingual (Spanish/English) preferred. Must pass a criminal background check and drug test.
