Nebraska Medicine is seeking a Heart Transplant Coordinator to join the Transplant Program team, this role is located in Omaha, NE. the Heart Transplant Coordinator will be responsible for post transplant clinic coverage and triage.

Job Duties: Transplant Coordinator
Provide and promote the delivery of 24/7 transitional quality patient care for a designated group of patients through role modeling, displaying excellence in nursing knowledge and clinical practice, and acting as educator for the transplant patient population. Provide a continuum of care and education for pre and post-transplant patients and their families (living donor or recipient) and serve as a patient advocate in the healthcare system and community.
